LETTER OF INFORMATION

Dear Fund User,

You and the Independent Living Fund (ILF)

I am writing to tell you that as from 1st October 2017, payments to you from the ILF will be paid from the new Independent Living (2016) Fund ("the 2016 Fund"). This is a new Trust Fund set up to replace the existing ILF Funds - the Independent Living (Extension) Fund and the Independent Living (1993) Fund ("the Old Funds").

Existing Users will automatically transfer to the new 2016 Fund and therefore do not need to do anything. Payments will continue in the usual way, except that they will be made from the 2016 Fund from 1 October 2017.

At the moment you receive your ILF money from one of the Old Funds. The change to the 2016 Fund is intended to consolidate and help our administration of payments in the future to all Users. We believe it is in your interest for the transfer to take place as the Old Funds will cease to make payments from 1st October 2017.

The existing terms of your ILF award and the obligations owed by you to ILF will also remain the same, except that now they will be owed to the 2016 Fund with effect from 1st October 2017.

Your personal information, which is held by the Old Funds, will be transferred to the 2016 Fund and will continue to be used only for the purposes for which you provided it.

Your continued acceptance of the payments made to you from the 2016 Fund will constitute an agreement to the change from the Old Funds to the 2016 Fund.

You will also find enclosed with this letter a form to advise us of any respite and/or hospital admissions. Under the terms of the 2016 Trust Deed the Fund must be informed of any admissions into respite care and/or hospital.

If you have any questions about this letter, your ILF payments or your personal information, please get in touch in the usual way by one of the means detailed overleaf.

Yours sincerely,
