Agreement Form

The Agreement form is the form completed by someone applying to the ILF (or a person on their behalf) once they have had their assessment and received an offer of funding.  Completion and return of the form means that the applicant (or their representative) understands the purpose of the ILF award, how it was calculated, how it can be used and their responsibilities as someone receiving money from the ILF.  Payment of the ILF award usually starts when the Agreement form is received.

Some people have asked for the Agreement form to be available on the website so that they can print it off on their own printer or obtain a new copy if they make a mistake filling it in or mislay the original.  The form is available as a pdf document or a Word document.

If you have any questions about how to complete the form, please contact your caseworker in the usual way.
